Hi Rage.
I'll try to explain this as clearly as I can. 
To start off you need at least one graphic editing program which can read .act palettes, such as Photoshop.
Then you'll need STI-Edit, you can easily find it in the modding tools list in this forums.
Say you want to add a gun image:
So start off by opening Sti-Edit, open up your Data 1.13 folder, look for the Interface Folder, open MDGUNS.sti and save the Palette in photoshop format (.act).
Now, still in STI-Edit, open data 1.13, go to tileset folder and open up smguns.sti, and save the palette in Photoshop format, like you did before.
Now, you need three images, a big one, for the Infobox, a medium one for the interface, and a small ground one.
Once you've created the big one, save the palette and the image as a .bmp file.
On STI-Edit, create a new document, set your palette, open the .bmp and save it in "BigItems" folder as GUN[lastgun+1].
I.e. GUN784
Now, for the interface one, open up your MDGUNS palette in photoshop, design your gun, and save it as a .bmp
Then back to STI-Edit, open up MDGUNS.sti and scroll to your last gun. click on the arrow with the + symbol on it, and load your .bmp.
For the ground item, it's the same process, the only difference is you'll have to add it to smguns.sti
If you want to edit objects, the name of the .sti's is MDP1ITEMS.sti and SMP1ITEMS.sti.

How do you use this color tables? How to open them in photoshop for example?

Sorry for not replying. I can't access photoshop right now, is there any way to do that in gimp? Seems newest GIMP can only import palettes, but can't export them.
And BTW, what are the max sizes for each category -big, medium and small? Is there any specific reason/limit for these? Any width/height combo permitted as long as its below certain size? Seems when i try different sizes, the image is offset. And offset does nothing for items AFAIK, at least i can't see any influence on my game.
