I killed Mike at the central SAM (Below the Cambria mine).
The result was loss of all town loyalty in every liberated city.
Why?

The only customization was the inclusion of different music. This would not change anything else though.
I was in the central SAM site below Cambria. I took out all enemies besides Mike and three red shirts.
I finished the battle.
After the battle, all of my liberated towns: Omerta, Chitzena, Drassen, and Cambria were next to zero.

Sorry for the confusion.
All enemies were killed including Mike.
First, I threw tear gas at Mike. It affected him. Reaper get's up to throw a stunner and notices his old pal. Mike asks if we have anything to say...we do...blah...blah...
3 mercenaries run at Mike. We want to steal his H & K G-11. Mike shoots a merc. We go in and steal his weapon. Then we stabbed him to death. Mike's dead.
There were 2 enemies left in the tear gas trapped building. Kill Kill.
Done.
Look at the strategic map. Loyalty plummeted globally.

Actually I have never seen a mustard gas trigger in that sector.
No civilians are in the SAM sectors and no militia were trained before hand.

Okay, I do not understand the Mike issue, but I have another one.
When I hit the switch for the secret room of the barn in sector I11, it results in the continuous stop watch animation.
I have one lone mercenary in the sector.
