I quickly looked at the files in the "src/video/quartz" folder in SDL's package and found references to NSQuickDraw functions. I found more of them if the windowed code than in the fullscreen one, but it might be that I missed some. The Quartz port must be only partial, and it will probably never be improved in SDL 1.2 since it's updated only for bug fixes and SDL people focus on 1.3 now.

So you can't expect something better from SDL. I think I read (on SDL's FAQ) that the windowed mode on Mac OS X takes twice as many passes (and time) than on any other OS. But I don't believe it can explain such a slowdown on the 640x480 screen that stracciatella uses. They say it's not slower than other OSes in fullscreen.

Regardless of SDL, I can have a major slowdown of stracciatella when the executable is built with the JA2TESTVERSION and/or JA2BETAVERSION and/or JA2EDITOR options enabled in the "config.default" file.
