With NCTH, barring save abuse, there will almost always be a chance to miss, though application of more bullets can overcome this. Point is, no 100% chance of critical head shot point in the game without significant limitations.
I generally advise a tweaked expert or tweaked insane, depending on how much ordinance you want to be spending amongst other things. With normal BR and drop all off, unless you are planning on planting serious boots to the back of various heads by night then stealing everything, expert is probably about the right level of weapons escalation.
Either way, on those settings, making sure you get the longer range weapon from the guy by the west sandbags (preferably by knocking him out and stealing gun and ammo) in A9 will make getting to better weapons a lot less painful.
