Thank you for the comprehensive reply. It did help me. I had read the official mod descriptions and a couple of threads about the diseases but none of it could prepare me for the absolute severity of it. Actually, I am not 100% sure if the mod is working as intented, unless it's intent is to ramp up the difficulty way past what the game can otherwise offer (even on insane) because it sure as hell isn't even remotely realistic in the current default form, which I had expected it to be considering the great quality of all the other mods.

Here's what happened. It makes for a great story and was a memorable gaming experience anyway. I searched for the skyrider right after freeing Drassen like I always do. Buzz was showing symptoms in the skyriders cabin. I diagnosed my whole team. Everyone had Arulcan plague and malaria and most had something else too. Five days after stepping on Arulcan soil! Stephen had something like this; malaria, typhoid, cholera, tetanus and arulcan plague. I took the SAM site that was next to skyriders cabin and flew my team back to Drassen. Less than 18 hours after Buzz started showing symptoms everyone was dead. Danny, Fox and Beth was nowhere near enough. Ok. Load game. I play on soft ironman (it is self imposed because I don't like to use the ironman mode due to somewhat frequent crashes). I only load when something ridiculous happens, like it did now.

I decided to turn off the disease just for a day to give me a slightly more time to prepare. I hired more doctors Mitch and Needle, and more medkits, and then I turned on the disease again and boy was it though. New diseases kept popping left and right. I hired Eli and Bernie too. All in all this is what it took me to survive.

EIGHT best doctors in the game working 24/7.

TWO different places to treat the mercs. Drassen Airport which was showing some signs of disease and Omerta rebel base where I was transporting the mercs who no longer had the Arulcan plague so that they wouldn't catch it again. Bernie and Needle were finishing the treatments there. Bernie used the diagnose now and then and more diseases kept popping even in the quarantined Omerta base, but luckily nothing life threatening. Everyone was wearing masks and gloves, doctors and patients both, but still the doctors were getting diseases too.

THIRTY medkits were used in the process.

TEN days it took for me to get my mercs cured.

TWO of them still died. I had to abandon the worst cases Stephen and Rev so that the others could survie. Otherwise Ira, Carlos (I play with early rebels), Dimitri, Moses and Snake would have died too.

But none of this would have been enough if my IMP wasn't a spy. As soon as she was free of Arulcan plague she travelled to Balime apothecary.

THREE seperate trips to Balime for cures to cholera and Arulcan plague were also needed.

This truly is a game on a difficulty far beoynd insane. Luckily I was playing a custom game where I had more money from the start to begin with and this time I had turned off the Drassen counter-attack. I had just completed another campaign a few days ago and I wanted to try something different, more realistic this time. Most of my tweaks to .ini were things that removed minor issues that had annoyed me in my last run. I had doubled the weapon damage, weapon range and visibility which makes the game a lot more realistic. Even the smallest of battles need to be taken seriously when your mercs are only a one or two bullets away from death. It seems I should have feared diseases more.

I am not angry at the mod, the idea is great and mostly the execution too because it forced me to use methods like the quarantine for the worst cases. Pure genius! But... it's still way, way too harsh. Maybe I had a bug though I hadn't touched the disease settings or maybe it's meant to be this way, but I wouldn't recommend playing with the diseases on.

I took a look at the diseases parameters but they seemed complicated and I wasn't sure if they were save game compatible, so after my mercs survived the ordeal with two of them dead and almost all of the money gone I turned off the disease altogether. I'll focus on the fights from now on. It's going to be an uphill battle, but that's what makes the early game great because by the midpoint game has always gotten drastically easier.
