In the newspapers today, covering the death and funerals of a TV anchor woman:

A friend of the deceased went to order a funeral wreath and asked that it should be inscribed Rest in Peace, on both sides. Later she picked up the order, only to discover at the funeral she was carrying a wreath nicely inscribed with:

Another common one I see in newspapers and mags lately - when an article is mentioning something or some bit of history it's followed by (check facts) but the writer didn't find the time to remove the note to himself, which makes me wonder if they even checked the facts as well.
